During the first visit, we make sure to obtain important background information and give you time to get to know our staff. It is our goal to make your visit with us an extremely pleasant one and to send you home feeling better than ever.

Below is our New Patient Forms that you will need to complete on your first visit. You can expedite matters by downloading the package, completing it at home and handing it to our staff when you arrive for your initial visit to our office.

Insurance Information

PLEASE UNDERSTAND. With Affordable Health Care constantly changing we are trying to maintain our credentialing with all current plans and to gain access to additional plans. You MUST contact the office with your insurance information so we can verify if we are currently on your plan.

We accept many insurances including AETNA,  Humana PPO  and United Health Care for chiropractic services. 

Traditional Medicare and Some Medicare Advantage programs are accepted for chiropractic services.

Contact us to determine if your specific plan covers chiropractic care. 

We are also included in your personal injury auto policy for accident related injuries. Remember, you only have 14 days after your accident to begin treatment or your PIP benefits will expire.

We offer cash programs and financing to fit your budget if you have no insurance.
